FAQ’s

  • We will get to know each other. We will review your medical history, answer your questions, and perform a physical exam. This will build a comprehensive understanding of your health profile and establish a baseline for your future care. 

    Our mission is to provide the best care in the most efficient way to get you to live well today; so each visit may differ in order to address and keep you on track to live better tomorrow.

  • We recommend answering these questions ahead of your scheduled visitLog into our patient portal and tell us about your medical history, medications and other relevant information we need to make sure we make the most of your visit with us. 

    Here are some things to bring with you:

    • Photo ID

    • Insurance cards

    • List of current medications and dosages

    • List of allergies 

    • Relevant medical records or test results if available

  • Life happens, we know.  So, if you are unable to keep your appointment, call us within 48 hrs prior to your scheduled visit.

  • Please call the practice ahead and let us know you are running late. We offer a 15-minute grace period when that happens. Unfortunately, we cannot guarantee to see you if you arrive later than the grace period.

  • Just email us using your patient portal. We’ll get back to you with the best solution for your request within 24hrs.

  • You can view them on your patient portal. If there are urgent concerns, we will reach out by phone or email. We’ll discuss all of the routine results at your next visit.

  • Send us an email on your patient portal or give us a call. We will direct you on next steps.

  • Please allow 7 business days for completion of all forms.

  • Email us on your patient portal or call us ahead of your surgery date.

    These forms need to be completed no earlier than 30 days prior to surgery and there may be additional testing or referrals you need. These appointments are scheduled based on availability. So, plan ahead and give yourself enough time to work with us on completing your form in time.

  • If it is urgent, call us. We will respond to all non-urgent matters within 48 to 72 hrs.

  • All prescriptions will be sent electronically to the pharmacy on file in your chart.

  • As a general rule the practice does not routinely initiate or refill opioid medications for chronic pain management. We will collaborate with your pain management specialists to create and maintain a comprehensive plan for your chronic pain.

  • We crafted wellness plans to suit the different needs of our uninsured patients. Please visit our services page. If you need help deciding, just give us a call. We’ll help you choose which plan will help you live well today and even better tomorrow.

  • We accept money orders and credit/ debit card payments.

    We are a cashless practice.

  • We accept private health insurance plans, Medicare and Medicaid.
